1. About these terms
These Terms and Conditions govern use of the Lifebulb LLC (“Lifebulb,” “we,” “us,” or “our”) website and the scheduling and care text-messaging program described below. Please review these terms before using the website. Where we ask you to accept these terms through a checkbox or similar action, that acceptance applies to the identified website feature. These terms do not replace your clinical informed-consent documents, telehealth consent, financial agreement, Notice of Privacy Practices, or other service-specific documents.Nothing in these terms waives rights you have under applicable health care, privacy, consumer-protection, or other law. A general website acknowledgment is not consent to optional texts, optional cookies, marketing, or a disclosure requiring a HIPAA authorization.2. Services and availability
Lifebulb is an outpatient mental health practice offering therapy in New Jersey, Virginia, Pennsylvania, Delaware, Georgia, Texas, and Massachusetts. Psychiatry and medication management are offered in New Jersey, Virginia, and Pennsylvania. We provide telehealth care and in-person care where available.Substance-use-related treatment at Lifebulb consists of talk therapy. Lifebulb is not a residential rehabilitation or inpatient substance-use treatment facility and does not provide emergency detoxification or medical withdrawal management.Availability depends on clinician licensure or other legal authority, your physical location at the time of service, clinical appropriateness, scheduling, and other applicable requirements. A state listed on the website does not mean every service is available in every location or from every clinician.3. Emergencies and urgent needs
Lifebulb is not an emergency or crisis-response service. Website forms, appointment requests, email, text messages, and portal messages are not monitored continuously and must not be used for emergencies. If you are in immediate danger or need emergency medical attention, call 911 or go to the nearest emergency department. Do not wait for a response through the website or messaging system.4. Website information and the treatment relationship
Website articles, assessments, screening tools, clinician profiles, and other content provide general information. They do not diagnose a condition, recommend a treatment for your individual circumstances, or replace advice from a qualified clinician. A screening result is not a diagnosis.Browsing the website, submitting a form, or requesting an appointment does not by itself establish a clinician-patient relationship. A clinical relationship begins through the applicable intake, acceptance, and care process. This does not diminish privacy obligations that apply to information you provide before becoming a patient.Do not start, stop, or change a medication or treatment solely because of website content. Clinical decisions are made with your treating clinician. No particular outcome, diagnosis, prescription, medication, or insurance reimbursement is guaranteed.5. Telehealth and medication management
For telehealth visits, tell your clinician your actual physical location and an appropriate callback number. Notify us before an appointment if you will be in a different state. Care can proceed only when the clinician is legally permitted to treat you in that location.Use a private setting and an appropriate device and connection. Telehealth can involve technical interruptions and privacy risks and may not be appropriate for every situation. Your clinician will address informed consent, emergency planning, alternatives, and next steps if a connection fails or a different level of care is needed.Medication management requires an appropriate clinical evaluation and follow-up. Prescriptions, refills, monitoring, and any controlled-substance prescribing are subject to clinical judgment and applicable law. These terms do not promise that a particular medication can be prescribed through telehealth.6. Appointments, fees, and insurance
An appointment request is subject to confirmation. Provide accurate contact, location, and insurance information and notify us of changes.Fees, insurance participation, patient payment responsibilities, cancellation and no-show terms, and billing procedures are provided through the applicable service and financial documents. These website terms do not create an undisclosed cancellation fee or other charge.Information about insurance is not a guarantee of coverage or payment. When you are uninsured or choose not to use insurance, you may be entitled to a Good Faith Estimate. See our Good Faith Estimate notice or call 855-722-4422 for assistance.7. Minors and authorized representatives
An adult submitting information for someone else must have authority to do so. Care for minors is subject to applicable consent, custody, confidentiality, and parental-access rules. Where a minor may legally consent to a service independently, we follow the applicable law. The general website is not directed to children under 13 for independent use.8. Scheduling and care text messages
Program and scope. If you choose to enroll, Lifebulb may send recurring texts concerning scheduling and care, including appointment confirmations, reminders, scheduling changes, and care coordination. Messages may be sent using automated technology. We do not send marketing or promotional texts.Your choice. Text-message consent is optional and is not a condition of purchasing services or receiving treatment. Enrollment requires your affirmative choice through our consent process; supplying a phone number alone is not our substitute for that choice. You represent that you are authorized to receive messages at the number provided and will tell us if that number changes or is reassigned.Frequency and charges. Message frequency varies with your appointments and care needs. Message and data rates may apply under your wireless plan. We will coordinate other appropriate communication methods for your care.Help. Reply HELP, call 855-722-4422, or email support@lifebulb.com for help. Do not use HELP for an emergency.Privacy. To the extent applicable law permits, the general informational website is provided on an “as available” basis without a warranty that every feature will always be accurate, uninterrupted, or error-free.This statement applies to website operation and general educational content. It does not disclaim professional standards of care, obligations under a treatment relationship, privacy and security duties, or liability that cannot lawfully be limited.12. Changes and contact
We may update these terms and will identify the effective date. We provide additional notice or obtain renewed agreement when required. Revised terms do not retroactively change clinical or financial obligations, replace consent required by law, or waive existing rights. If any provision is unenforceable, the remaining provisions apply to the extent permitted by law.
